Once you’ve browsed our site to find the best fit therapist for you, you can fill out our prospective client form to provide us with all the background information we need. A member of our administrative team will reach out to you to get the scheduling process started.
STEP 3
COMPLETE INTAKE FORM
After scheduling your appointment, a member of the administrative team will send you a link to our client portal. Within the portal you will verify your demographic information as well as complete any intake documentation prior to starting services.
